to I need to hand in my option c and my T4s?

just wondering im confused about this. It seems that the guide is very confusing to me and when I call in to ask it seems I get 4 different answers from 4 different people. Do I need to hand in both option c and t4 or just option c?

Give them as much as you can. The option c form is not always representing the last 12 months.
