One of my friend has visited Thailand on short period visitor visa 7 years ago and he lost his passport.
He has new passport now and doubtful about exact dates of visit to Thailand? How he should complete foreign travel
history form to be send with other doucments or he should leave the form blank with no travel history. Seniors please advise

Concealment of fact is not advisable.Please ensure if ur friend has any of the followings:
PHTOCOPY OF LOST PPT
COPY OF FIR LODGED WRT LOST PPT
BOARDING CARDS OF FOREIGN TRAVEL
ANY RECORD OF FOREIGN EXCHANGE PURCHASED FOR FOREIGN TRAVEL.
Foreign travel form should be submitted with required information nd the facts wrt lost ppt ,which his new ppt should essentially reflect it ,must be highlighted in the cover letter.
Though not an expert, yet I am of the view that the issue may be sorted out as above.Others are also requested to help the needy friend with more practicable solution. Thanx.

Only month and year is required to fill the travel history form. May be he can get the same from the e-tickets available in his emails etc.

(a) if you leave the travel history Form blank, it will be like concealing truthful information
(b) if you fill the Form, and no passport to verify the same - Department can make it a chaos

Very confusing situation. (Better to get a written advice on this from the immigration department and not to follow the guesses by blog users)

Hello Friends,

I am in great confusion and dilemma. The following is the exact copy of email I received from NDVO. My confusion is that I am the principal applicant and I am Male. The letter they addressed is Ms. XXX. I have three daughters in my application. That means one of my daughters is required to take the additional medical test, because the family name mentioned is mine. They have not mentioned clearly which of my daughters has to take the test. I have written them a detailed email asking them to please specify which of the daughters in my application has to take the additional test. Its been a week and no reply from NDVO. I also went to PP(Panel Physician) where I did my medicals, but he is saying that unless it is specified which daughter has to take the additional test, he can't refer me to a chest specialist. What to do now? Shall I go and get the additional tests done for the entire family.

Did any one got treatment of TB in past if not sure take Chest X-Ray and meet to any doctor they can say whos case needs to be re-medical. If any one has white spots on lungs its basic symptom of TB or healed TB. It will just delay your case but will not be a reason of refusal of visa.

1) Any one in my family has not been treated for TB

and

2) I am not from punjab, i am AP
 
Get a digital x Ray done, There will be some spots on your chest , some time pollution related spots are mistaken as TB

If no particular name is mentioned ,all of you will have to Undergo additional tests.

Take the letter to the DMP and they will be process the needful

HI KDSTTL,

Do not leave travel history form blank it will not be right.. But you can surely right the tentative dates of travel and write a note that
though these are not exact dates explaining your situation in your case this would be (loss of Passport)..

Get Chest Xray done from local doc.. i am sure he will be able to tell you which one of your kid needs re-medical mean time keep sending reminder
to CIC NDVO they have yet not closed for vacation.

Adding more to our Guru jee's advise , you all family members may go for digital xray n visit a chest specialist with all xray reports. He will tell you who is under suspicion of CHC . You may then approach PP with that family member for further evaluation.
