Other question I had; when asked for any other name (nickname, alias etc), should I give them the artist name under which I create music? If so, should I keep the other 'family name' box blank, or should I still give my family name in that specific box?

Put your given name and family name in the boxes that ask for them, and use the names as they are in your passport.
Under the 'other names' section, I would list your artist name. If the last name of your artist name is also your family name, this won't matter: just list the full artist name in the 'other names' section, and list your official given and family name where they are asked for. Don't leave the 'family name' section blank.
