my wife doesn't have an income,but she is getting 4000$ monthly as a trust...

Send the empty Option C and attach other documents proving income. And, on a separate sheet of paper, detail her situation.

The Option C will prove that she was not on welfare. The other documents will prove she has the financial means to take care of both of you.
