No need to get in to these details. They don't matter at this point. Just prepare the documents and explain the situation. Obviously they haven't looked deep enough at AIP level which they are now getting back to. It doesn't mean that it's all bad. VO is providing OP a chance to explain this situation, since he already started a PhD at one place and trying to understand why is he switching to another one. VO formally asking for an explanation and based on that, going to judge that whether OP is going to switch to another program in Canada, or even worse, abandon studies and try to stay illegally in Canada. That part comes under admissibility, not eligibility.

But it seems he also had an outgoing PPR, I thought that only happens once you are approved ?

No and yes. System generated stuff once Eligibility is cleared. But that doesn't kick in until its Final = approved.
