i formatted it in a word document. one event per page. i have about 30 pages. 2-3 pictures per page.

what details should be included?

That's exactly what I did... same amount of photos, too! Except for my Wedding/Reception/Honeymoon photos, those are loose and seperate.

For my captions, here's an example:
Left: xxx (Sponsor) and xxx (Applicant) with xxx (Applicant's Sister) and xxx (Applicant's Sister's Boyfriend) at [insert location] on [insert date]. We had a wonderful time shopping at the mall and playing in the arcade. He won a stuffed animal for me :)

hmmm... why were those pictures separated?
 
They were already hard-copy photos, but the rest of mine are digital! Plus, I have so many wedding photos I wanted to show (10-15 Wedding/10-15 Reception/10 Honeymoon) compared to the 2-3 per each event for everything else.
 
i sent approx. 60 pics (including the wedding) of our relationship. we seaparated them in two envelopes and behind each photo we wrote the date, the event, and the people appearing on the photo

Include names, dates and places. For dates, the month and year should be good enough, but we included the exact date because it was easy to find on our digital photos. The important thing is to document that you went places together, that you met each others friends/relatives, and that you have had frequent contact over an extended period of time.

I think we included about 150 pictures (haven't actually counted) over a 27 month period. For some people that would be a lot of pictures, but I have about 6100 pictures from the last 2 years (not all of them involving things we did together, because we were actually in separate countries for much of that time).
