Guys this form needs to be filled by principal applicant and spouse.

my problem is that for travel details in last 10 years, it has just 5 rows, whereas I have made around 30 trips in this period. how should I make so many entries on the form.

Also can I skip the travel details as per my last passport and show only the ones on my present passport ( from 2013)

Well the next set of instructions tell you to:
List all trips you, and if applicable, your family members have taken outside your country of origin or of residence in the last ten years (or since your 18th birthday if this was less than ten years ago)....

So if your current PP has all the travel entries covering the last 10 years - then you don't need to add old PP entries.
But if you have multiple PPs, check and the entries in those PPs for the last 10 years to get your travel history and add only that (i.e from 2006 - 2016).
