My parents' supervisa recently got approved - it is valid for a few years, whereas medical expires in a few months. What happens if their first travel date is after the medical has expired? Do they need to do a new medical, and does the duration of stay (whether it is more or less than 6 months) matter? And what about subsequent visits?

You have to renew supervisa insurance every year and purchase another year of supervisa insurance. If your parents have had medical issues both IRCC or the insurance company can ask for your parents to do a medical. If a parent has had serious and expensive medical care the insurance company may not be willing to renew the supervisa policy. No issue travelling 1 year after a medical has been done. Don’t worry about the medical once you have a visa or permit. If you are ever required to repeat a medical you will be notified. Most will not require one unless they apply to extend their stay or apply for a new visa or permit.
