Can someone please tell me which forms my father need to submit for SUPER VISA application.

I am aware of IMM 5257 ( Application for Temporary resident Visa)
IMM 5645 (Family information form)

But I am confused about IMM 5257 Schedule_1...

Do he have to submit that as well????
