My wife and I received emails from LDNIMMIGRATION@international.gc.ca last week. We've been advised to submit my (Principal Applicant) passport to VAC - Pakistan. Further, a scanned copy of a letter titled ISBAD VAC PHEP 2016 is attached with the email. The letter covers 4-Steps to performed prior to submission of the package (inclusive of the passport)

Step-2: Passport Requirements – Determine if you need to submit your passport(s)
Step-3: PREPARE YOUR DOCUMENTS
Step-4: SUBMIT YOUR DOCUMENTS
Step-1: Change in Circumstances
We have already intimated about a change in our personal circumstances (individually and a number of times) i.e. my wife (my sponsor); gave birth to our baby boy on June 09, 2015. As per Canadian Citizenship Rules (as listed on the webpage http://www.cic.gc.ca/english/citizenship/rules/), our son is a Canadian citizen as he was born outside Canada after April 17, 2009 and his mother (my wife and my sponsor); was naturalized in Canada before his birth i.e. February 24, 2010. So, we are applying for his Proof of Citizenship followed by an application for Canadian passport.
Step-4: SUBMIT YOUR DOCUMENTS
We've been advised to fill out Appendix A - Updated Information Chart and send it over with the package. We were curious to know if we should fill in the details of our son and my wife (my sponsor).
1. Please advise on what information needs to be filled in the chart?
2. Should I be sending over photographs of my wife and our son in the package?
