Study permit and TRV renewal. HELP!!

i am international student from india. i land in canada in may,2015. my program of study is 2 years long. My passport is going to expire on feb,2016, so my visa and study permit is only till jan,2016.
So i renew my passport. now i need to know how to apply for the renewal of the study permit and Visa.
Which documents i need to submit? Which form i should fill? kindly help me..... i shall be thankful to you.

1. Once you have a new passport, then you apply for an extension of your student permit: http://www.cic.gc.ca/english/study/study-extend.asp
2. Once you have the new study permit, then you can apply for a new TRV: http://www.cic.gc.ca/english/information/applications/visa.asp?countrySelect=CA#applications
