I am helping my father and mother to apply for extension online but I am stuck
They both came in June, 2012 - STAMP on Passport with NO DATES
When filling out the form for my father, it clearly had option of extending stay with date last entered Canada ( like 4 months ago ) THEN it ask if there is family member too? so we pick YES for my mother as spouse
Issue
1) The form says Date first entered Canada?
Now she visited in May 2001 for the first time and her last entry ( was with my father 4 months ago )
So I put May 2001 or the most recent
2) I have to pick an option for Extension, but the options are

Visitor Record Extension
Is is Visitor Record? I though it was a piece of paper with definite date to leave Canada? or is it the last option? Visitor Record Extension?
I have to select something to move forward
