i am trying to seek why im scheduled for an interview in beijing. so in order to fill out the form above, my questions are

1. should i check 'privacy act' since im viewing my own info? should i ask for copies or go view it myself, which way is faster? (i live in toronto)
2. under 'request for', should i check 'capis(info abroad)', or overseas visa file, or both?
3. should i mail only this form and where should i mail it to?
