what is the difference between these two questions?
I was refused visitor visa to Canada and student visa to US. What should be my answer?

Yes to both.

Q1 refers Canadian immigration
Q2 refers to all other immigration including Canadian.
 
In addition as you're answering "yes" to a statutory question it is recommended for you to provide a Schedule A along with your eAPR to save processing time as it might be requested later on.

It is not mandatory at all but recommended for anyone who answered "yes" to a statutory question. It saves time as it is common for the officer to request such form during processing if one answered "yes" to a statutory question.

Yes, merge it with your LOE.

Schedule 4 if you've been nominated by a province.
