Statutory Questions Confusion

Has ................ been refused refugee status, or an immigrant or permanent resident visa (including a Certificat de sélection du Québec (CSQ) or application to the Provincial nominee Program) or visitor or temporary resident visa, to Canada or any other country?

Has..................ever been refused a visa or permit, denied entry or ordered to leave Canada or any other country?

what is the difference between these two questions?
I was refused visitor visa to Canada and student visa to US. What should be my answer?

Q1 refers Canadian immigration
Q2 refers to all other immigration including Canadian.

In addition as you're answering "yes" to a statutory question it is recommended for you to provide a Schedule A along with your eAPR to save processing time as it might be requested later on.

Is schedule A mandatory for everybody?

It is not mandatory at all but recommended for anyone who answered "yes" to a statutory question. It saves time as it is common for the officer to request such form during processing if one answered "yes" to a statutory question.
