Statutory Declaration

I am not able to get Referral letter from previous company then my friend suggested me to get Statutory declaration from colleague of equal designation or higher management. For that case I need sample letter format.
 
What application is this in regards to? You may be better off posting in the relevant section of the forum if its for say the FSW scheme. As a side issue CIC will want to know why you can't get a reference letter...their initial position tends to be one of suspicion.
