Statutory Declaration of Common Law Union 5409.. do i need to fill this also?

I need to Apply for a Study Visa... I need to clear that in Document Check list there is a form for Statutory Declaration of Common Law Union 5409.

What is this for do i really need to fill this form?? I have my marriege certificate with me. Through serach what i get to know you have to prove that you are living in a relationship??

Other then this If My Wife is sponsering me for studies she has to declare on any official form or she just need to submit her Bank Statements??
