Hello,

Can I download and use this PPTC 132 form from the below notarypro website link(please see below)? One of the notary pro officials advised me to do so

Asking because, on the Canada.ca website(please see link below), they are asking to get it from the nearest passport service location or by contacting them via web form. Have raised a web form but have not heard back from them.

The form edition is 2017 July[PPTC 132 (07-2017) ], not sure if there is a latest one, if so this might be expired?

If the notary is giving me the form, I'd just go ahead take it since they would know the process quite well.
Lots of forms used in the process are pretty old, I believe some of the RQ forms are older than 2017.

Good Catch. I confirmed with Notary Pro customer service and they said it's okay to use this form.
Can someone here please confirm the latest form number for Statutory Declaration in Lieu of Guarantor (Form PPTC 132)

Has anyone recently got this form from the passport office? Do we need to wait in a queue to get this form or do they provide it immediately? Is this form available at service Canada?

This is the correct form as of January 2024. I just picked it up from service canada for whoever lands on this page from google.
