Statuatory Declaration

Hi All,

I have received a medical request for a non accompanying dependent. I am sending a Statuatory declaration stating that I am unable to get the medicals done of the dependent due to custodial reasons. Can anyone please let me know how and where I should send this documents. Is it via email or physical mail ? I have so far received two emails one for AOR , one for Biometric , both of them from “Do Not Reply - Ne Pas Repondre” email and medical request came in my CIC account. I am not sure where to send these documents. Any help in this matter is highly appreciated. Thanks

Would also include proof of any attempts to get the medical done that have been refused by the other parent.
