Thanks for that!! I'm sponsoring my spouse, and he lives in the Dominican Republic (well actually we both live there)... Really you need to re-do the medical exam if the processing takes longer than a year?? With most cases I've heard coming from here, the time is generally 12-18 months...

Yes, a lot of people do have to re-do their medical exams. In some visa offices, they ask for the medical after they start processing. You'll need to check the region-specific guide for instructions.
