I'm currently compiling the documentation required to extend my stay as a worker but am a little unsure about what exactly is meant by the stamped receipt IMM 5401.

I have a passport stamped by Canadian immigration when I first entered the country and received my work permit.
I have a K21 document issued by a BSO from the same time, stapled to which is my receipt.

Neither of these is labelled as IMM 5401. Which is required, or do I need something completely separate?

IMM 5401 is a reciept of payment form from CIC.
ANd this is what you use to go to a bank or western union to pay the fees,

Kinda like paying a utility bill. there is a thing at the bottom that the teller stamps when you pay, that means they have received that payment.

You have to order this form through the CIC website. and they will send you one through snail mail.

Which may take a long time.

If you make an account on the CIC website and pay credit card, you can just send a copy of the receipt sent through your email.

Also you can apply the extension online, in which at the end you can also pay through credit card.
