Hello I have an Open Work Permit, because I came to Canada acompanying my spouse who was an international student, but now she has applied for her Post Graduate Work Permit and also she is working in a ful time job in NOC B , so I would like to apply for an extension of my OWP I called to CIC last week and they informe this:

You can apply for an open work permit if you are the spouse/common-law partner of a post-graduate student who holds a work permit under the NOC skill levels O, A or B.
To apply for a work permit, you must:
Apply online on our website; or
Send an Application to Change Conditions or Extend Your Stay in Canada as a Worker to the Case Processing Centre in Vegreville, Alberta;
the application is available on our website.
Please note that additional documents are required for this application.
You must do this before your status expires. You keep your current status under the same conditions, until a decision is made (implied status).
So, according to the official of CIC who attend me I have the requirments, but I wanna know which form I ned to submit IMM5710E ??where can I explain the exemption c41 and attach the post graduate work permit of my wife?does anybody knows?
