Spouse sponsorship

Hi, I am applying for PR as an Individual and thinking of applying for my spouse later once I have the PR. The reason for that is as my spouse has undergone kidney transplant 3 years ago so someone has suggested to proceed with the application later.

Please suggest me as what will be better for me to avoid any rejection of PR
1. Applying in together for PR
2. Applying it later once I have the PR.

What I know is in both options the medical examination should be done by both of you before you get the PR even if you choose the second option

Is that for express entry?

It doesn't look like a question for spousal sponsorship.
As MuAn said, if you are applying under EE or provincial nominee, both you and your spouse have to go through the medical exams and police clearance. It doesn't matter if you apply with your spouse or apply first and put your spouse in "non-accompanying" and apply for spousal sponsorship later.

The difference is depending on timing, your spouse may need to get her medical exam twice. Once for yours, once when you sponsor her. (exam result is only valid for a year, I believe)

So does it mean that even if she is applying without me still there are chances of Getting rejection due to my health issue

Exactly! They have to look at both of you. Because if one is allow to become PR, the spouse is eligible to be sponsor (regardless of the medical condition). That's why they need to know that for the whole family (spouse and dependents) before they approve PR of the applicant. (regardless if the spouse and dependents are coming with the applicant right away or not)

Do not exclude yourself and try to "trick" the system. If you were excluded from her PR application, she is NOT allow to sponsor you forever. And she will be at risk of her own PR status because of misrepresentation.

So is there any way where I can give assurance to the government that I will bear the medical expenses also in case of future kidney problems, I will be acting as a donor. If yes then how much chances are there to get the PR

I do not have the answers. Try to search for "Medical inadmissibility" and in your section of immigration stream (EE or PNP) and see what others do. As I stated spousal sponsor is VERY different.
