Spouse Open Work Permit Questions - Help Plz!

Hello im an Indian living in Montreal having PGWP and doing Job here. Recently I got married and would like to bring my spouse on open work permit. I went to the cic website and reviewed eligibility, answering the questions as some one who would like to get the spouse in to Canada. Atlast its showing to upload the forms which are imm1295e, imm5257b, imm5707e of which imm1295 is the application form. My question is since im applying for my spouse, I have to fill in her info in the said form right? and also the supporting documents I can append to this form and upload right?

1. You complete the forms as if you are your wife, as she is the one that is applying for the TRV and OWP.

Hi thanks for the reply and the additional docs I can attach to the imm1295 form and submit?
