hi, i applied my spouse open work permit visa application in january , and then now we got a medical request but we got a call from delhi immigration to do medical furthur but we received medical request form is incomplete only birth date and UCI number given other all blank information so how do we contact to immigration to recorrect medical request form ?plz reply me ASAP
