hello KMAEP..ok lang..satistified nako sa sagot mo thats enough for me friend..so san kn ngyon? sa Canada knb?..ganun nga din gagawin ko kc from Lucena City ako pag isahin kn lakad ko sayang kc pamasahe db? san ba location mo dito? and san destination mo?...sa Wed ako punta ng cfo for seminar and sticker..

thanks for the reply sis! binuksan ko kasi yung b4 na form eh ang dami pang fi-fill up-an, eh ang onti naman ng dadalhin ko puro damit at pasalubong kasi yun ibang impt na gamit nadala na ng asawa ko nung umuwi sya last year.. ;) wala din akong goods to follow.. hehe! thanks sis!

Kapag sa nationwide makati after a week..
May ibang clinic. same day nakukuha agad :)

copy 2 of the medical report section A (IMM 1017) yung ibibigay sa'yo :) yun yung may signature ng DMP hindi yung Appendix C :).. 2 copies yung dalin mo na Appendix C .. Isa sa kanila , yung isa ibabalik sa'yo kasama na ng Copy 2 mo:)
