Spousal sponsorship Application requirement

I am applying from outside Canada in the USA to sponsor my spouse into Canada. I have filled out the sponsor application and we have filled out the applicant application forms. Do we need to include an application for Permanent residency card at the same time and pay the fee for that up front as well?

No, you won't apply for the PRC until landing.
