Sponsorship for Wife- Planning to submit application by Sep 2014

We are planning to submit the sponsorship application for my Wife by sep 2014. Have some questions regarding the same
1.Do we need to submit upfront medical checkup, along with the sposnorship application

Yes I am applying outland, But will be staying in canada on bvisiting visa for couple of months
