I am self-employed, but I do not have a business name. Nor was my business ever established. How do I answer those questions?
…to clarify, I am a coach. I am registered and certified by Skate Canada (a National umbrella for figure skating). I coach at a business (a skating school), but I am considered self employed when billing and for tax purposes.

Please help!

I would put N/A for name and the date you started doing it. Include a note explaining the situation.
