there is no imm5002,,i have been looking i didnt find it,,,and are you gonna sponsor your spouse inside or out of canada?

Be sure to also get the appropriate region specific forms (IMM 3901 - IMM 3913). You use ONE of those. In there, you will find a checklist that the applicant needs to fill out and in some cases, additional forms that should be included with the application.
The 'E' letter at the end of the form number is just to signify the form is in English rather than in French (meaning IMM1344FB is the same form as IMM1344EB, only it's in French). You can choose to send in either the forms in French or English.
Besides the forms, you will also need to include the payment receipt for the processing fees and of course any other documents that are required for the application, such as the medical exam form, police certificates, document copies etc.
