Spelling Mistake In Medical Request form

Hi,
Im my medical request CIC has witten my daughter's name wrongly Instead of JOANN LIYA JOSEPH wrote JOANN LILA JOSEPH. One single mistake, this can be corrected by DMP. I am going for Medicals tomorrow.

Please anyone with such previous situation comment..I am having such mistake now..My daughters Middle name not spellt correctly and my contact number is not correct as well. seniors shine a light on this please.
