I wanted to know if anybody received similar request.
"A report from a specialist Paediatrician and/or developmental Psychologist is required regarding applicant's cognitive and developmental status. Please forward a report addressing history, diagnosis, managment needs and any social services requred ".
Background : My son and I had our medical exam where we had declared about his autism diagnosis. During the medical exam, I shared the most recent devlopmental pediatrician report ( 4-5 months old) and the latest IEP from school.
Question :
1. I want to know if somebody received a similar request and if you can please share what details you included while responding to it ? If you can, please share your template or redacted version of report or provide any pointers
2. Should the developmental pediatrician include a brief summary of the history and recommended services or they should also mention the scores for each section of the evaluation test ?
