Address those refusal reasons in a letter of explanation
And Keep your SOP attached as well. Maybe you need to revise your SOP since you got refused.

Its a personal choice. If I was in your place, I would write two separate letters.

If you do not disclose your previous refusal and act as a fresh applicant, you may get rejected again for not disclosing it. Secondly, you need to justify each reasons given in your refusal letter along with evidence/documents.
