Is it necessary that the “Additional Family Information” form to be filled by the accompanying spouse also?? Or it must only be filled by the principal applicant.
If the dependant should also fill it, what does he/she write in the first section, where it is written:
Name
Relationship (which has the options: Applicant, Spouse, Mother, Father)
Dt. Of birth etc.
Where is the dependant supposed to include his/herself: as the applicant? Or the spouse? I mean in the case when the spouse is the person filling the form.

Please help.

In my application, Me and my wife filled-up this form separately.

For me (principal applicant)
Applicant: My Name
Spouse: Her name
Father's Name: My Father's Name
Mother's Name" My Mother's Name
Children: Our Children's Name
Brother/Sister Name: My Brother/Sister's Name
Signed by me

For my wife (as spouse)
Applicant: My Name
Spouse: Her name
Father's Name: Her Father's Name
Mother's Name" Her Mother's Name
Children: Our Children's Name
Brother/Sister Name: Her Brother/Sister's Name
Signed by Her

Although there's no checkbox to indicate if the form is for applicant or spouse, I think it's the immigration officer's job to distinguished what is what.

It's better to also wait for other members opinion regarding your question.

I made some correction, and i think which is the correct approach which i already followed..........

Remember that both you and your spouse are applicants, even though you are the *only* principle applicant.
Hence, your spouse's name should be put in the applicant name box in her form.

I disagree with this... I believe that the principal applicant's name should be the same on both applicant and spouse form. The principal applicant is NOT the spouse and the spouse is NOT the principal applicant.

How about if you have children more than 18 years old and they also need to fill-up this form. What they will put on the form under Applicant Name, their own name? I don't think so.

Yes Giogenre....even children above 18 have to fill their own forms. The applicant in their form will be their name, not principal applicant's.

Before I filled up this application, I asked a lot of forum members and they told me that the principal applicant's name will be common to both me and my wife's form (which is also my own interpretation). I respect your understanding on this but I guess I will believe on what I understand on this form. Well, it's immaterial now because I already submitted my application. Only the immigration officer really know what is right. IMHO. Thanks! ;D

So, you're saying that you put the principal applicant's name under "applicant on both forms? Did you encountered any problem with that? I mean, did they accept your application?

Yes, put it on both forms.

Since, my application was recently received by CIO (November 29, 2011), I cannot tell at the moment if there was a problem in my form answers. I think they are still reviewing September applicants. If ever my answer is wrong I hope it will not be a reason for them to return my application, it might be to minor to be considered a fault (IMHO).

Because we have conflicting advice to you, I suggest to wait on other member's opinion on this before finalizing your form. I'm not claiming what I did is correct, I just believe in my own interpretation of the form. :D
