so Q1 - tick A if ur still with the same employer

Then on Details of intended work
Q- what type of woekr permit ur applying for - choose other and then fill Bridging open work permit next to that.

You can still change whats on ur form... all u need to do is reupload again after any changes

Im guessing u only need to pay it if u apply for a work permit after Feb 21

As long as you have an open work permit, the employer doesn't have to pay the compliance fees...

No you dont need to, It is clearly mentioned in the document checklist that they need your recent T4 at the time you applied. whcih was 2013 T4 so ur good.
Unless they ask you they want 2014 T4 keep it framed on the wall :P

I think it won't hurt to submit it. I am also planning to send it once I get my NOA. My experience counts from May 2013-September 2014

Usually CIC don't look into T4 alone. CIC using T4/NOA to see if it's matching each other and salary stated in your reference letter. I suggest you not to send additional documents until they request it (or if you see some concern raised by VO in GCSM notes).
