Yes. I was really thankful for this forum.That's why ahead of time before my PPR arrives, i have gotten my AOM already :)

If you still have your dutch DIGID, you can login into it, submit, pay and let it send to you (you can give a adress where they will send it).

I already have an international birth certificate. When I tried to copy it, it didn't copy the edges of the document (letter size is smaller than A4), so since I was talking to CIC anyway while I was scanning and copying, I asked them if they in fact needed it. The document checklist, after all, used "or", implying it would be enough if I sent one of the things on that part of the list. When the CIC agent told me it was not needed, I didn't include it.

It seems that about 50% of answers I get from CIC agents are dead wrong, and that annoys me. >:(

Anyway, I will wait for Berlin to contact me and see if they request it (they probably will). If they need only a copy of my birth certificate, I can scan it at Fedex (where they can scan something bigger than letter format I assume) and just email or fax it to them. If they need the original, I will authorize my dad to order one for me, and have him mail it to Berlin.
 
I received AOR from my visa office confirming they received my app on Sep. 2. Let's see how long more it'll take them for the next steps.
