Separation Declaration for Minors Travelling to Canada [IMM 5604]

Hi,
I am planning to apply for FSW along with my minor kid, if other parent of child is not accompanying to canada, need to submit a signed statutory declaration from non-accompanying parent. I am living separate from husband and we dont have any contact with that person, so I am unable to produce signed form. Could please any one advice how i should i proceed in this situation.

Then you are going to have to get a custody order from the court allowing you to remove the child from his/her home country.
