Seniors Pls Help- IMM0008 Application form Current country & Spouse status

I am filling my IMM0008 generic form.Stuck in two places.

1. Current country of residence- I am ICT employeer transferred to Canada from January 2011- Novmber 2011 and transferred again on March 2013 .
What will be my From date in Current country of residence? January 2011 or March 2013? Pls help
2. My spouse is in Open Work Permit and She never worked. Whats her status will be "Visitor " or "Worker"?

1. Country of residence - country where you are living at the moment. Canada, I presume
2. If your spouse has work permit, then her status should be "worker".

What will be my "From date" in the Current Country of Residence???
My "2013" Entry or "2011" Entry?
"To date" I put my expiry date of WP
