Section A of Medical Request

Hi All,

There is a field provided in Section A to provide contact person in Canada. I pressume this must be the relative I have declared in my application???

Also, this form should only be filled up at the DMP right?

There is nothing to do with a relative in Canada for the medical. You don't have to enter anything there.

Guys correct me as well. I didn't do anything in the medical form sent by the CIC..i just handed that over to the DMP thats it...was there any thing that needed to filled up by me and/or my spouse?

You don't have to do anything with the medical form.....just hand it over to the DMP.

I am going for medical as well can you tell me if the pictures that they request for medical exam are the same size as the ones sent with full docs?
