Second Stage (Visa Office Specific forms)

Hello All
I have not yet received the AOR from NS but I have started collecting all the necessary documents. I want to make sure that I do everything right and for that to happen, I am posting all the necessary documents that I need and have collected up to now. All of yours eyes will certainly help me check my documents and all of your's feedback will surely make my case stronger. I will really appreciate if the experts can input some suggestion so that the newbie like me get it right. Some of the questions I will be asking may sound stupid but I am doing this just to be 100% sure on what I am doing.


1 Form
Statements: I am supposed to make the photocopies of all the original documents that I have send to NS.
Questions: Do I need to notarize this entire document?
My answer: Negative. If someone has different answer, pleases post.
Confusion: Visa office specific forms states “Send notarized photocopies of all documents except the police certificates, which must be originals".

2 Identity and Civil Status Documents
For this, I have Marriage certificate, my US driver license and Social Security Number
Question: Will this be enough? Do I need some more documents? Do I need to need to notarize these documents?

3 Children’s Information
For me it’s inapplicable

4 Travel documents and Passport
For this, notarize passport copies should be all I need. Any thoughts are welcome.


5 Proof of Relationship
My brother is a Canadian PR and he has sent his copies of PR and landing documents. He is sending me his lease agreement.
Question: Does photocopies of Canadian PR and landing documents needs to be notarized? If yes, then I believe my brother needs to do this in Canada and send me the copies through mail.
Any thoughts: I was thinking I do not have to as these are documents possessed by Canadian Immigration. Also, I was thinking instead of lease agreement, can I print his credit card statements or else his phone bills that show that he is in Canada? If I can print it online, how should I notarize it?


6 Educations
My understanding is they need the original sealed transcript from the university and certification of completion. Do I need to notarize the certification of completion? Also, my understanding is that sealed transcript cannot be notarized.
Any thoughts…

7 Work Experience
The visa office specific forms states “notarized employment contracts from your present and past employers”. I have the contracts but my question is how I am supposed to notarize from my employers.
Original and up to date letter of reference: Again, do I have to notarize it? The problem is, it will be in the envelope.

8 Proof of Language Proficiency
My understanding is I need to send original results of the IELTS.
9 Arranged Employments
Not applicable for me
10 Non Accompanying Family Members
Not Applicable
11 Settlement Funds
Bank Statements and Bank certification letters
Question: Are these documents needs to be notarized? If yes, does one bank does notarization of different banks and also how are we supposed to notarized 6 months statements?
12 Police Certificate
Per my understanding it is originals as stated. Any other views….

13, 14 and 15 which are respectively Fee Payment, Interview location and photo requirements
I do not have any problems and confusion.


Sorry for this long question but I hope all of yours input will make it easier and accurate for all the newbie like me who are confused on some of these stated documents. Any input will be highly appreciated.

Make photocopies/or type info exactly same as in previous forms,sign them and put recent date, I hope u may not change any other details in these forms, there is absolutely no need to notarize these forms

U should include BIRTH CERTIFICATE, other wise CHC will demand it later, all documents must be notarize

Provide them your Travel history list(includes entry /exit dates)
all pages of your passport. Notarize copy
Copy of Visas. Notarize

No need to print these documents online. Request ur brother to send notarized copies of all of his documents including these bills( may be original) etc.

Just request ur university to attest/authenticate photocopies of your transcripts & degree certificate and then get these copies in sealed university envelope. If u submit these attested copies in University sealed envolpe then no need for any further notarization.

Just notarize copy of your contract. Notary public will do it, no need to get it certified from employers.
Send original letter of reference, written on company letter head, also send business card of the person signing that letter. No need to send any notarized copy as it would be the original copy which u will need to send

Should be up to date, 3-6 month old bank statement, on bank letter head and stamped by bank officials. You have to go to all banks (in which u have acconuts) and Bank officials of each bank will certify your account balance statements, Bank officials will not certify ur other account statements from different bank, they will care for only those accounts which are in their branch. No need for any other notarization
12 Police Certificate
Per my understanding it is originals as stated. Any other views....

It should be original.

I do not have any problems and confusion.

Answers are based on those cases who already got their Visas, I have personally gone through their cases, so am sure about these things

do you want to mean that the bank statement should be 6 months old .... lets say I will send the Visa office specific forms on March,2910 so ... do i have to send a bank statement which was given from the bank on October, 2009???

would u plz explain main pages/all pages of wat?? if u r asking about Passport then only main pages needed to be notarized, but u have to send copy of all the passport pages to them , even blank ones

Thanks Guys but a quick question, if I make a clean photocopies then the signature will already be in the photocopies with the older dates…. Correct. Looking back at the paperwork, I found out that there is no signature place on the main application but is on the background declaration page which is on the end, so like you have said I can make a clean photocopy of the last page and sign it with the recent date. Similarly there is no signature place on the schedule 3 forms so no problem on that. But the additional family information is a single page with three signature place; do I have to make a clean photocopy and then retype it and sign it with the recent dates. What do I need to do? Looking for some more clarification from you guys.
