Please use the following link to provide details of your ADDRESS history for the
period OCTOBER 15, 2005 TO JANUARY 3, 2006.

 Do not leave any gaps in time.
 This must be received at this office by: 2015/07/23

My qustion is do I need to compelete the form or just fill the address?

Complete the form. Schedule A request is sent if they want you to explain any gaps in your personal history like they've done here.
