It appears the forms have been updated.

Here it states to use eIMM 5669.

I same dilemma question I don't know which form I should Send for Spousal Sponsorship?

The form finder states which form is to be used. If you are sponsoring your spouse why are you looking at the guides for parents and grandparents? There are instructions for how to open forms both on this forum and on CIC. I do not have time to link to them now. Sorry
