Hi All!! I am yet to submit my PR application but I just noticed from the forum that since I have one of the statutory questions 'yes'. I guess I will submit an Upfront Schedule A IMM5669e Background/Declaration in order to avoid ADR and dont want delay in my application.
My question is as follows:
1. I am using the Form IMM5669 (06-2019) E. Is that the correct one?
1. In question 7. Education: What should I give as 'Field of study' for primary education? Should I put None?
2. In question 8, Personal History: in the question "If you were outside your country or territory of nationality, indicate your status in that country or territory?". I know that I do not need to add travel details in personal history for the PR application, however can anyone confirm if I need to add travel details in Schedule A Question 8 (Personal History) or is it similar to the entries I have added in PR application Personal History?
3. In question 8, I have overlap of dates due to part-time job I worked while studying, is that ok?

I would like to have @legalfalcon @13nitinsharma @srvp and experts advice too!!

Sch A is only required when specifically as led by IRCC.

Not everyone gets a Schedule A request.
 
Thanks for reply @legalfalcon . Since I have answered "Yes" for one of the statutory questions, I would like to have it prepared upfront so that I can have faster application processing.

YEs, this is the correct form

You can list it as secondary school / high school or Middle school as the case maybe.

The Sch A is also a part of the application you submit. Sch A is asked in case of any discrepancy or clarification. In the personal history list all activities including travel and if required attach a new sachet of paper at the end.

Thank you for detailed response. For address history, should I add addresses of hotels I stayed while touring other countries too?

I don't remember one of the tour hotel address I stayed in Oman..
Currently I have only mentioned addresses of my long stay like home address while working at home country and US address where I did my Masters. Will that suffice?

When I entered the details for personal history and in the 'City and Country' it goes beyond the lines and + sign appears. When I print the form, the + sign appress hiding the letters. Please refer to the image below. How do I fix this?

Also, there is no validate button in this form, I can only find "Save form", "Reset form" and "Print Form". Am i missing something?

If you have too much info, you can enter it on a plain sheet of paper. This form does not require validation.

Its normal to not remember addresses of your short stays. You can mention and explain it in LoE.

I have been filling the statutory questions & schedule A.. based on my history I was denied OPT back in 2015 and got an rejection for my earlier PR application 2020. Should I say 'yes' to both the questions below:
1)"Have xxx been refused refugee status or an immigrant or permanent resident Visa or visitor or temporary resident visa, to Canada or any other country?" - yes. Give explanation for both OPT denial and PR application rejection
2)"Has xxx ever been refused a Visa or permit, denied entry or ordered to leave Canada or any other country?" - yes Give explanation for OPT denial. ( Should I add PR application rejection explanation too?)
Schedule A
6 d. been refused refugee status or an immigrant or permanent resident Visa or visitor or temporary resident visa, to Canada or any other country - yes, Give explanation for both OPT denial and PR application rejection.
e. been refused admission to, or ordered to leave, Canada or an other country or territory ? - No.(as I have never been denied entry or deport in anyother countries)

Is this correct way to express?
