Schedule A - Background Information

Hi all,
I have a silly question, I am the principal applicant and I am filling the "Schedule A - Background Information" form for my wife. At the end of the form it says "Declaration of the applicant" and "Signature" so my question is who should sign this particular form, me or my wife?
