On #4 it asks for details of my father and well my situation is very complicated, but to make a long story short, I have never met my father (who's on my birth certificate). The man who has raised me since I was born has always been there. Whose details do I put down?

Note: I have no details of my "birth" father, just his name on the certificate.

If legally he is your step father, you would go with step father name, and if i was you i would attach a separate sheet with that form to explain the situation in detail so they wont get confuse and ask you to re-do the form.....
