scarborough office, Signing a form during the interview

Hi There,

I did my citizenship test and interview today. During the interview, an officer asked y to sign a white form. I did not understand what was that? Any idea or experience?

By the way, I failed the test. Do you know how long the rescheduling take time?

I signed the form too... I passed and have received oath letter.

Thanks, the form was given to you during the interview by the officer? (when he was reviewing your application and documents)?
