Reusing Custodian Declaration for Minor SP

I am a minor trying to extend my study permit from last year. I will have the same custodian. I want to know if I can reuse the initial notarized custodian declaration form from my first SP application, or if I need to fill in a new declaration. Thx

There are 2 items that come into question. It asks for the school you will be attending, are you changing? Going from secondary to post secondary? The second is will your custodian still live a "reasonable distance" from where you will live and study. If you are changing schools you should probably have a new one is my opinion.
