Restoration of Status Application Form

Hi Guys,

I need a bit of help on my situation to restore my status. I recently got rejected from my work permit extension, and my temporary resident status expired Dec 19, 2024. My work permit expired Oct 13, 2024. I am restoring my status to be a visitor, I am not sure on these two questions,

What is my current immigration status?

When did the status expired?

Should I be putting the Temporary resident status expiration?and whether should I put my status as visitor?
