If applying for PR, you cannot say you will provide and pay for your own healthcare. You must undergo and medical and be medically admissible to Canada (not a burden on the healthcare system)

Coming on a TRV, then you want travel/healthcare insurance for any medical emergencies.

what is "self Healhcare without help"? Step 1 is entering as a visitor since that's your friend's plan.
You need to be prepared for trips. You cannot predict accidents...etc.
Hospital stays or any treatment in Canada is very very expensive. We had to pay for my MIL cos her emergency health insurance didn't cover most of the treatment she got when she was visiting.

Self healthcare is not possible for PR. He must pass the medical and not be medically inadmissable. If he is then he will need a Canadian immigration lawyer to show that any care now and in the future will not burden the health care system. So if he needs specialists, hospital stays, surgeries etc. that will be considered. Assume he has completed his ECAs, IELTS to submit an Express Entry profile, and is eligible.
