Requested Medical Report-Section A Client identification and summary(IMM 1017E)

Hi !!

I submitted online application for study permit on November 28, 2014.

Today i received an email in which they asked for additional document Medical Report - Section A Client identification and summary (IMM 1017E), though i have already submitted my e-medical tracking sheet while files my application.

I talked to hospital they said my medical which was done on 9th of November has already been sent to embassy.

No clue what to do, please help!!!!!

Applied online via MyCIC account on cic.gc.ca website.

And they haven't any email address except one for queries, my application doesn't evn have an upload option for document.
