Request for medical exam IMM 5753 (12-2012) E

Dear everyone:
I am applying for the study permit and one of the required document to upload is the "request for medical exam" form:

I need a medical exam, but I do not need the instruction letter from the consulate before I see a panel physician because I do an upfront medical exam. So should I choose yes or no in the form?
